You have several options for turning in forms.  If you belong to a church that is part of the coalition, you can give your form to your church representative.  If you are not a member of a church in the coalition, please print out the forms and complete them.  Then you can turn them in by faxing them to 801-848-9743, mail them to PO Box 243, Simpsonville, SC, or
